Bookkeeping for therapists and counseling practices
A therapy practice can have a full calendar and still wonder where the money went. Insurance payments arrive weeks after sessions, copays come through cards and cash, and contractor clinicians need to be paid from what is actually collected. Mispar reconciles reimbursements, platform payouts and payroll so the numbers match the practice. Plans start at $229/mo.
Best for: Solo therapists, group counseling practices, psychologists, social workers and multi-clinician practices using an EHR or practice platform.

How Mispar keeps your practice books clean
Industry-specific accounts and categorization, with the reporting scope agreed before work starts.
Insurance reimbursements and adjustments
Match remittances to sessions where records allow, and keep contractual adjustments and denials separate from real revenue instead of booking billed amounts as income.
Copays and card payments
Reconcile card processor payouts, cash copays and platform collections to the bank, with fees on their own line.
Contractor and employee clinician pay
Keep clinician compensation tied to collected revenue where that is the agreed structure, and reconcile payroll or contractor payments each period.
Session packages and prepayments
Where your policy and accounting method call for it, track prepaid packages apart from revenue recognized as sessions are delivered.
Practice platform and telehealth costs
EHR subscriptions, telehealth tools and billing service fees stay on distinct lines so software creep is visible.
Owner pay and practice overhead
Rent, supervision, continuing education and owner draws stay organized so the practice's true cost of running is clear.
A simple, predictable process
From kickoff to your first clean close, here is exactly what happens.
Review EHR and payer reports
We start from the records you already have: practice platform reports, remittance advice, processor statements and bank feeds.
Map revenue and cost lines
Payer payments, patient payments and clinician costs are mapped to an agreed chart of accounts with opening balances reviewed.
Reconcile payments and payroll
Deposits, processor payouts, remittances and payroll records are reconciled to the bank each month.
Close monthly books
Monthly statements with an exceptions list and the provider-level reporting agreed for your practice.
Illustrative example
An insurance remittance covers many sessions from weeks ago, minus adjustments. We record the payment against the right period and keep the write-offs visible, so last month's deposit is not mistaken for last month's earnings. This is an illustration of the method, not a customer case or a guarantee.
Counseling & Therapy Practices bookkeeping — your questions
How do you handle insurance payments that arrive late?
We reconcile remittances to the periods the sessions belong to where records allow, and keep adjustments and denials visible. The goal is books that explain the delay instead of hiding it.
My clinicians are 1099 contractors paid a split. Can you track that?
We can organize clinician splits around collected revenue where that is your agreement, reconcile what each clinician is owed, and keep the payment records clean for year-end forms.
How should prepaid session packages be recorded?
It depends on your accounting method and refund policy. On an accrual basis, prepaid amounts are generally tracked as a liability until sessions are delivered. We follow the policy agreed with your CPA.
Will my EHR or practice platform replace the accounting file?
The platform explains sessions and claims; the accounting file explains the financial statements. We review the exports available and reconcile them with the bank.
Does the $229 starting plan cover a group practice?
The final plan and scope depend on clinician count, accounts and transaction volume. We agree that scope before starting; catch-up work is discussed separately.
